The terms “her,” “she,” and “Holomua” refers to Holomua Healing Arts, LLC (Britt Yap).
The terms “Client(s),” “they,” and “their” refer to customers, clients, and any other financial investors of our services.
Client agrees to pay Holomua the fees for each service provided, including all taxes.
Client agrees to pay for the services at the rate listed on the Holomua invoice and/or Holomua website. Client must pay the 50% deposit fee prior to the session either by phone or through Holomua’s website. Client may choose to pay the entire session in full prior to the appointment. Client may request for an invoice and/or receipt and Holomua will provide that within 3 days of the request. Holomua accepts cash, credit card, or a valid Holomua gift certificate as methods of payment.
Sessions will be delivered by meeting at a specific time and place agreed upon by Holomua and Client. If Client cancels an appointment with less than 24 hours notice (by phone only), Client is responsible for 50% of the session fee. Therefore, Holomua reserves the right to keep the 50% deposit fee. When Client reschedules their appointment, they need to pay another 50% deposit for that scheduled session. Holomua retains the right to make exceptions for what Holomua deems an emergency situation. Cancellation should be given by phone (call or text 1-832-819-4325), not email, if cancellation is within 48 hours of the scheduled appointment.
Holomua reserves the right to cancel an appointment if she is not feeling well enough to perform the service at 100%. Holomua will always do her best to give Client notice a few hours before the scheduled session or a day in advance.
Client’s identity, relationship, and content of the sessions are strictly confidential except in a situation where such confidentiality would violate the law or cause harm, or unless Client gives Holomua written consent or verbal permission to share the content of the session to help others (ex: workshop, speeches, book).
1. The Relationship Between Client and Coach
Holomua will partner with Client on a journey of discovery, self-exploration, accountability, and follow through. Holomua will ask the Client questions in order to help them discover ideas and goals, aid them in creating a plan, and hold them accountable in reaching their goals. Holomua helps the Client to move forward and progress on their goals and encourages them to start living their ideal life.
2. The Relationship Between Client and Hypnotherapist
Client authorizes Holomua to hypnotize them for the purposes outlined in this intake form and for the future purposes that the Client requests. Client understands that the success of the hypnotherapy depends greatly on their own ability and desire to effect change in their life. Client understands that the results of their sessions depend greatly on their own serious participation, and that Holomua cannot offer any guarantee of the success of their treatment. Client is aware, however, that Holomua will do everything in her power to ensure the Client’s success. Client also understands that they have other choices from which to seek assistance regarding their specific concerns, and they have chosen hypnotherapy at this time. Client understands that during the hypnotherapy session, Holomua may touch the Client as an anchoring technique.
3. The Relationship Between Client and Reiki Practitioner
Client understands that Reiki is a form of complementary healing, which enhances the body’s ability to heal itself. Client also understands that Holomua is not a licensed physician. Reiki is a complement to ‘healing arts services licensed by the state’, i.e., it is a complement to more traditional western medicine provided by doctors, nurses and hospitals. As a complementary or alternative medicine, Reiki does not require licensing by the state.
Client understands that during a Reiki session they will lie down or sit seated, and be fully clothed. Holomua places her hands on or above the body and allows the energy to radiate from the hands. Client understands that stress, trauma and illness restrict the natural flow of Ki or Chi life force energy through the body. By restoring Ki to the body, Reiki rejuvenates the body’s ability to relax and de-stress, and supports its ability to heal itself. It is a way to ‘tune-up’ your energy to its healthiest state allowing for healing to take place.
In addition, Client understands that they may feel heat, energy, and other sensations throughout their body during and after the Reiki session. Client understands that during the session, Holomua will be radiating the Reiki energy through her hands, sessions may involve Holomua sweeping her hands above the body in various ways, or incorporating breath or vocal tones to break up energy blocks.
Client understands that Holomua cannot offer any guarantee of the success of the Reiki treatment, but knows of the positive benefits that may come from just one Reiki session.
Achieving the results desired and expected by the Client and Coach normally takes time. In order for coaching to be most effective and to allow time for changes and results to occur in the Client’s life, the Client understands Holomua highly recommends the Client to commit to a period of 3 months or more.
Achieving the results desired and expected by the Client and Hypnotherapist normally takes a few sessions. In order for hypnotherapy to be most effective and to allow time for changes and results to occur in the Client’s life, the Client understands Holomua highly recommends the Client to commit to a period of 3-6 sessions to start.
Achieving the results desired and expected by the Client and Reiki Practitioner varies. Some Clients have seen success in just one session. Other Clients prefer to see Holomua anywhere from 2-4 times a month, once a month, or several times a year in order to get the most benefits from Reiki. Client understands that often times Holomua receives messages and has visions during the Reiki session. If Client consents, Holomua will share her visions/messages with the Client. It is up to the Client on how they interpret those visions/messages and what they want to do with it. Client has the right to ask Holomua not to share visions/messages she receives during the session.
Holomua offers a free 15 minute consultation phone call prior to the Client’s first scheduled session. Holomua may also schedule 15 minute phone calls prior to coaching and hypnotherapy sessions to check in with Client in order to prepare for upcoming appointment. However, unscheduled calls or sessions where the Client calls Holomua will be billed at the regular rate of $100/hour after a grace period of 10 minutes.
All products and services are sold “as is.” All services, workshops, and products are non-refundable. If a Client purchases a product and it does not fit, Client may exchange the product for a different size in the same exact product if Holomua has that size in stock. Holomua reserves the right to make exceptions for emergency/special situations.
Client agrees and understands that Holomua is not a medical or physiological professional. Holomua is not a substitute for psychotherapy or medical practice. If a situation arises where medical or psychological intervention is needed, Holomua is ethically and legally bound to refer the Client to a mental health or medical professional.
Client shall indemnify Holomua from any and all damages, liabilities, costs, losses, expenses or attorney fees arising out of any claim, demand, or action due to sessions with Holomua. The services and products of Holomua are sold “as is.” In all circumstances, the maximum liability of Holomua to Client for damages for any and all causes whatsoever, and Client’s maximum remedy, regardless of the form of action, whether in contract, court or otherwise, shall be limited to the net profit of Holomua.
